Abstract

本研究是以質性研究之非結構式-深度訪談的方式,針對強迫症患者在面對疾病的同時,其休閒參與、休閒阻礙及其生活品質、環境狀況作探討。研究對象以北部某聯合醫院強迫症特別門診及認知行為團體治療中,找尋六位能以語言清楚表達自身感受和自願參與的患者及其家人進行研究。研究分三大部分:研究一,探討發病前與發病後個人和家庭的轉變,瞭解其家庭壓力及休閒阻礙因子。研究二:針對休閒阻礙理論所提出的三方面阻礙因素進行探討。研究三:探究強迫症患者在休閒運動方面,需要的支援有哪些,在其心中又扮演著什麼樣的角色。

研究結果發現,強迫症患者的生活問題與壓力來源大致可分為:疾病、家人和社會。在休閒阻礙方面,內在個人阻礙有:疾病、睡眠與體力、時間、懶散;人際間阻礙有:人際關係、人際壓力和缺乏休閒夥伴;結構性阻礙有:場地、經濟、舒適感,以及乾淨的環境與否。在休閒支援部分分為五大項:經濟、環境、休閒夥伴、心態與疾病,以及有形、無形的支持與協助。在休閒運動所扮演的角色方面,大部分的強迫症患者皆表示,已沒有多餘的心力去從事休閒運動,但對休閒運動仍有正面的想法,即便過程中存有許多自我矛盾,且同時與外在的各項因素間不斷拉扯與排斥。

The research uses non-structured in-depth interview to explore and understand Obsessive-Compulsive Disorder (OCD) patients’ leisure participation, leisure constraints, living quality and environmental circumstances while enduring the symptoms. The research subjects are six patients who can express themselves clearly and their parents. The patients and families are volunteers to participate in this research. All the patients are from one of the United Hospitals in the northern part of Taiwan. The research is divided into three sections. In part one, the research attempts to understand the factors of family pressure and leisure constraints before and after the symptoms. In part two, the research will verify the three constraining factors evolved from leisure constraints theory. In part three, the research explores the supporting elements of patients’ leisure activities and the role of leisure activities in their mind.

The research results discover that the problems and sources of stress come from disease, family and society. As for leisure constraints, interpersonal constraints include disease, sleep, stamina, time, and laziness. Interpersonal constraints are interpersonal relationship, peer pressure, and lack of leisure partner. Structural constraints include places, economy, coziness, and clean environment. The supporting elements for leisure can be divided into economy, environment, leisure partner, mindset and disease, tangible and intangible help and support. Most OCD patients express that although there is no spare energy for leisure activity, they still intend to have leisure. However, there is self-contradictory during the process that intertwines with external factors creating massive barriers and obstacles.
